Assumption of Risk and Waiver: I am aware that there are inherent risks involved in participation in a physical activity such as basketball. These risks include, but are not limited to, injury while traveling to and from the venue, injuries such as sprains, cuts, bruises, ligament damage, broken bones, strains, contusions, or concussion that may result from contact with the ball, floor, walls, or other participants. I agree to accept the inherent risks in participation of the activity. I hereby give camp personnel the authority to act on my behalf in case of emergency. I agree to hold blameless and to release and indemnify Creston Valley Suns BB Club, Kootenay River Secondary School, Club Coaches and Personnel from all claims, damages and expenses which may result from accident, injury, or loss by or to the participant during the program.
